Your letter intending to close the account will suffice the very purpose along with Check book, if you are holding. But keep Acknowledged Copy of your communication.
Advisable to specifically specify where to divert the Balance of the Account &/or withdraw to Max extend.
To reactivate the same account your application to the effect with some additional amount to be deposited so to get it reactivated.
In case of Chaeckbook holder, bank may impose penalty for not keeping Minimum Balance.
